Walk-In Hours & Locations

We will see patients who have had a sports injury within the last 24-48 hours (e.g., sprains, strains, fractures, etc.)

Eligible for any sports-related injury (e.g., football, baseball, soccer, volleyball, cheerleading, etc.)

Standard insurance rules apply – we must be in network with your plan (exception: if you have school insurance coverage)
- If the athlete is on school insurance, they must present a school claim form. Personal insurance plan will be entered as primary (regardless of plan) and the school insurance will be entered as secondary coverage.
For the treatment of a minor, a parent or legal guardian must be present or provide valid written consent.
- Written consent includes: child’s full name and date of birth, statement authorizing medical treatment, parent/guardian’s name, relationship to child, signature, date of authorization, and scope of treatment allowed.
